Transcript Requests

If you are a current or former Elmwood Park High School student who would like to request your EPHS transcripts, please follow the directions listed below. To access the directions that apply to you, scroll down.

Please note that diplomas are not available for re-order. Graduates can only order transcripts.

Current Students EXCEPT Seniors Applying to College

These directions are for current students who are NOT seniors applying to college. No fees are required for these requests. To obtain a transcript:

There is no charge for transcripts for current students who are not seniors applying to college. These transcripts may be needed for scholarship applications, driving school, Illinois Secretary of State documents, insurance discounts, etc.

Current Seniors Applying to College

These directions are for current students who ARE seniors applying to college. No fees are required for these requests. To obtain a transcript:

  • Students with last names beginning A-G:
    • Your counselor is Emma Chaplinksy, who can be reached at chaplinskye@epcusd401.org
  • Students with last names beginning H-Per:
    • Your counselor is Francesa Leone, who can be reached at leonef@epcusd401.org
  • Students with last names beginning Pes-Z:
    • Your counselor is Miriam Luna, who can be reached at lunami@epcusd401.org

Obtaining Transcripts

Graduates or Former Students can obtain transcripts. Please follow the instructions below and allow three days for processing.

  • Download and complete the Request to Release School Records Form ($5 Fee)
  • Make your check/money order payable to EPCUSD401 or write your credit card/debit number on the form with the expiration date and v-code
  • Mail, scan or fax the form and payment to:

Obtaining Test Scores

To obtain scores for ACT, SAT, SAT Subject and/or Advanced Placement exams, you must contact the ACT or the College Board directly.

If you attended EPHS before the 2009-10 school year, your ACT score is listed on your transcript.
