Transcript Requests
If you are a current or former Elmwood Park High School student who would like to request your EPHS transcripts, please follow the directions listed below. To access the directions that apply to you, scroll down.
Please note that diplomas are not available for re-order. Graduates can only order transcripts.
Current Students EXCEPT Seniors Applying to College
These directions are for current students who are NOT seniors applying to college. No fees are required for these requests. To obtain a transcript fill out the linked form:
There is no charge for transcripts for current students who are not seniors applying to college. These transcripts may be needed for scholarship applications, driving school, Illinois Secretary of State documents, insurance discounts, etc.
Current Seniors Applying to College
These directions are for current students who ARE seniors applying to college. No fees are required for these requests. To obtain a transcript:

- Return the form to the registrar or your counselor in person, via mail, email or fax; If you are not sure who your counselor is, see below
- Transcript requests for college admissions must be processed through Naviance

Graduate & Former Student Transcripts
Transcripts can be obtained through the Parchment platform. Please click the link below and follow the instructions to create an account and request your transcripts. Graduating classes prior to 2010, please allow for extra time in processing requests.
Obtaining Test Scores
To obtain scores for ACT, SAT, SAT Subject and/or Advanced Placement exams, you must contact the ACT or the College Board directly.
If you attended EPHS before the 2009-10 school year, your ACT score is listed on your transcript.
